There have been games that attempted to provide content within the original level cap. The first that comes to mind is Ragnarok (for a while at least, but then they added rebirth and things got weird).

Also, if we've been paying attention to the patch notes, the possibility of cap raises shouldn't be such a shock: a lot of items/instances/quests being added were marked as "for levels 50+" or some variation of that (at least in the original notes I read). The idea is that raising the level cap, and then filling in those new levels with added content, is probably the most fluid way to add material to the game. Isn't that what we wanted?
